我們聲明我們將遵守中國質(zhì)量認(rèn)證中心的認(rèn)證規(guī)則和程序，支付認(rèn)證所需的申請，試驗(yàn)， 工廠審查及其它有關(guān)的費(fèi)用；中國質(zhì)量認(rèn)證中心將不承擔(dān)獲得產(chǎn)品合格認(rèn)證的制造廠或銷售商應(yīng)承擔(dān)的任何法律責(zé)任。
We declare that we will follow the rules and procedures of the CQC and make payment for the fees arising from the application， testing， inspection and other services.

注：1、申請人應(yīng)將申請書寄CQC有關(guān)產(chǎn)品處。郵寄地址： 中國 北京市朝陽區(qū)朝外大街甲10號；郵編：100020
2、請用中、英二種文字填寫申請人， 制造商、制造廠和認(rèn)證產(chǎn)品的名稱。
3、 有關(guān)CQC產(chǎn)品認(rèn)證的公開文件可通過上網(wǎng)獲取， 網(wǎng)址：http：//www.cqc.com.cn
4、如果申請CQC認(rèn)證，同時(shí)也申請CB測試證書，請?zhí)顚懲辍吧暾埲顺兄Z”后，繼續(xù)填寫CB測試申請書。
5、CQC認(rèn)證與CB認(rèn)證同時(shí)申請時(shí)，制造商必須相同。
6、CB申請書以英文填寫為準(zhǔn)，相同的內(nèi)容可以復(fù)制。
申 請 人 承 諾
1） 始終遵守認(rèn)證計(jì)劃的安排的有關(guān)規(guī)定；
2） 為進(jìn)行評價(jià)作出必要的安排，包括審查文件、進(jìn)入所有的區(qū)域、查閱所有的記錄（包括內(nèi)部審核報(bào)告）和評價(jià)所需人員（例如檢驗(yàn)、檢查、評定、監(jiān)督、復(fù)評）和解決投訴的有關(guān)規(guī)定；
3） 僅在獲準(zhǔn)認(rèn)證的范圍內(nèi)作出有關(guān)認(rèn)證的聲明；
4） 在使用產(chǎn)品認(rèn)證結(jié)果時(shí)，不得損害CQC的聲譽(yù)、不得做使(中國質(zhì)量認(rèn)證中心)認(rèn)為可能誤導(dǎo)或未經(jīng)授權(quán)的聲明；
5） 當(dāng)證書被暫停或撤銷時(shí)，應(yīng)立即停止涉及認(rèn)證內(nèi)容的廣告，并按CQC要求交回所有認(rèn)證文件；
6） 認(rèn)證僅用于表明獲準(zhǔn)認(rèn)證的產(chǎn)品符合特定標(biāo)準(zhǔn)；
7） 確保不采用誤導(dǎo)的方式使用或部分使用認(rèn)證證書和報(bào)告；
8） 在傳播媒體中對產(chǎn)品認(rèn)證內(nèi)容的引用，應(yīng)符合CQC的要求。
